SDDT Happy, Healing, & Healthy (H3) Champion Description
H3 Champion: This person has a passion for making a difference in the community and can help fulfill CW’s mission by empowering and inspiring their networks and the local community in the area of cultivating healthy lifestyles. This role will help to expand the H3 curriculum into additional San Francisco Districts and support the H.E.A.L. (Healthy Eating & Active Living) focus citywide.
Specific responsibilities are to:
Have a cohesive understanding of the H3 program offerings and is able to complete the H3 Champion Certification Program (Stipend provided after the completion of the certification program)
Coordinate the registration and enrollment of participants to attend H3 “I Feel Good!” Program
Facilitate the “I Feel Good!” Program
Survey enrolled participants - Pre & Post Surveys
Weekly progress reports sent to the Program Director
Bi-Monthly 30-minute meetings with Program Director
General responsibilities of H3 Champion are to:
Check CW email regularly and respond in a timely manner
Follow all protocols laid out regarding your specific role
Complete all tasks according to the agreed timeline
Give updates regularly to the Program Director on your role
Keep updated documents organized in the designated folder in CW drive
Communicate needs, gaps, decisions to Program Director when needed
Qualifications:
Minimum Community Health Worker Certification/1-year experience as a Promotora / 1-year experience working as a Health Advocate
Must be a U.S. Citizen
Experience working with San Francisco's diverse communities and neighborhoods
Ability to work as part of a team to prioritize and handle multiple tasks, and to work
At least one year of group facilitation experience
Previous teaching experience a plus
Bilingual English/Spanish is a plus
Skills Required:
Strong people skills
Strong event coordination skills
Fluent use of Gmail email server for program correspondence
Comfortably use Google Drive as a home base program server
Ability to use the Zoom online Platform
Great communication skills with supervisor and coordinator
Compensation and Schedule:
Facilitated class is flexible depending on your approved schedule
20 hour H3 Champion Certification will be a set schedule and will be mandatory to complete for this role
Job Type: Contractor
Pay: $40 per hour for reporting, prep, and facilitation time. Roughly 20 hours a month.
